This is REV5 old thread........
I revised another version of this circuit on pcb. I made it even easier to build and it can be self mounted. I added some screw holes in case you did not want to use the pcb mount parts. I designed this to be as pro as the other companies out there.
The power connector is in EURORACK format.
The core circuit did not change. I did get rid of the Pre-amp. It was nice but I wanted to make it compact. The Bi-polar FM is still there.

The other IDC looking connector location is where you solder wires going to your jacks: BP out, FM in, etc. I use an IDC connector to another JACK pcb eliminating wires altogether.

I am waiting for about 15-20 pcb requests before I order them. everyone interested just post a reply how many you want and later I will remind all to gather the funds to make the order. The turn around is about 7-10 days for pcb arrival and shipping out.

I made a mouse BOM. This does not have LM4250 or ic sockets though. You need to get those elsewhere. Digikey carries LM4250.

This BOM also does not include the other (5) 3.5mm Jacks needed for the other inputs. You can use your own sources.

I really recommend to get the included 3.5mm jack in the BOM even though you might use this pcb for MOTM or 1/4" jacks. It serves as a strong pcb panel mount and you can use it to connect external 3.5mm gear. I have updated the pcb with another Audio In solder hole so if you were MOTM you can have a 1/4" jack also. On the picture above 'j6' has now become 'AIN1'

If this is your first DIY build and you own a EuroRack system I really recommend you try this out. You really can't mess up. It works at first power on, of coarse you have to turn the trimmer for proper Cutoff knob range. No measuring devices needed. It's adjusted by ear. Of coarse if you make a soldering mess and stuff the IC's wrong it will not work! Just be neat and patient.

Don't go mad trying to source the original UA Russian IC. The LM4250 has a flavor of it's own, respect it also. Just build and make noise.

No mods needed for 15v operation. There is a trimmer that can adjust for both formats. I personally have tried both 12v and 15v with no problems.
