Can anyone recommend where to get a kit with PCB and components or only PCB, to build a +/-15v and +5v?
Would like everything to be on one board if possible.
Should minimum be able to draw 1,2 amps.

All the DIY kits I'm aware of are +/-V only. Easiest way to get three voltages would be to buy a Power One/Condor/International Power supply instead of DIY. Buying good transformers in small numbers (one or two) is nearly as expensive.

I built this guy:

It works great and was very simple to build. Instructions are in depth. I take it you want to buy a pre-etched PCB, though? Also, you mentioned you already have a transformer. This design requires two transformers. One for +/-15V (36VCT) and one for +5v (9V). If you go that route, I can point you at the transformer I bought for the +5V if you like. I suppose you could use the 36V for the +5V as well, but it'd be wasting a lot of energy and maybe would generate too much heat on the the voltage regulator?
